For an erect abdomen you ask the patient to expire and stop breathing, with this breathing technique you should get the diaphragm at ribs 8-9.

This isn't happening with my patient who has a SBO. I take her xray everyday and no matter how many times we practice and try I still get her diaphragm at T 10. Is this just patient habitus? she is a very petite lady.

is there any other trick or sometimes they just have long lungs?

I Think you are doing the right thing, but you should not be doing xrays so much.
