For the facility at which I work, our couch can only approach so far into the gantry. If I tried to do an A&P head first, I'd probably get cut off before the symphysis pubis, on an adult, at least.
We often have to move pts down with their feet hanging off for a chest, if they are feet first. Our protocol for chests is for feet first...
