I have a question for somebody that hopefully knows the answer. When you are coding femoral angiograms with aortas and they put in a stent do you code where hte catheter was (distal aorta) when injected or where it was when the put in the stent and injected making it selected (internal iliac). In other words do I go with 36200 or 36246? HELP!!!!!!!!!!!!!!!!!!!!!!!!!!!!

Hi there,
To code for this procedure, if the physician injected contrast to the aorta and the iliac vessels from the same position, then you code 36200 only with radiology S&I code 75960. However, if the physician injected contrast in two separate locations for the angiogram, code 36246 for second level internal iliac artery with 75960. I believe this is right, I am a coding student. The 75960 procedure code includes the injections and angiography and completion angiography, according to CPT coding guidelines.

I work in an Endovascular lab and do all the coding for our procedures. As far as the procedure you would code cathetar placements (surg codes) and imaging (S&I codes). So if the phy had the catheter in the aorta 36200 and did an abd aortagram with r/o 75630 and then moved the cath to the int iliac 2nd order vessel 36246(on the opposite side of access) and placed a stent 37205 and 75960. you would code the 75630 the 36200 goes away b/c that is nonselective and you have a selective 36246. So you code the arteriogram 75630 and the cath placement 36246 and the 2 codes for stents 37205 and 75960. You do not ever code the completiong angiogram so there would be no code for that it is inclusive with the stent. Also new 2005 coding rule if you do and arteriogram then do intervention your S&I diagnostic code in this case 75630 would need a 59. I have more info on this if needed. Hope this helps! Laughing If the phy injected two seperate locations the catheter was placed in two seperate positions aka he injected from the aorta 36200 then the internal iliac 36246 you only code 36246 b/c of selective vs nonselective however the S&I you would code 75625 or 75630 which ever is needed and you would also code the S&I for the int iliac which is 75736 or something like that I don't have my coding info infront of me .
